Although the risk of embolism in atrial fibrillation patients has been substantially reduced with the introduction of oral anticoagulant agents, such as warfarin, there are still many patients who may not be optimal candidates for warfarin therapy. Furthermore, the monitoring of the drug is difficult and requires regular and frequent measurements of the INR, which is subject to intra- and inter-individual variations and requires technical expertise. The risk of ischaemic stroke in atrial fibrillation patients may be reduced by the use of aspirin, although the risks of bleeding, especially gastrointestinal haemorrhage, may also be increased. There are new oral anticoagulants, which have a more predictable anticoagulant response and may provide better patient compliance. New oral anticoagulant agents are being developed that offer a more convenient dosing and improved safety profile. Non-pharmacological and electrical cardioversion as well as catheter ablation of the atrial fibrillation may be useful in the future.
